Phase loss occurs when one phase of a 3-phase system is lost. This can be due to a fuse that's blown, overheating, broken wiring or loose contacts, or equipment malfunctions. When a phase is lost, extra current is drawn from the other two phases, which can cause overheating and damage to connected equipment.
